On Purpose Therapy operates within the social model of disability. We recognise that disability is a a difference, just like age, gender or race.
The fundamentals of how we work is by utilising a capacity building approach. We acknowledge that therapists are only present 1% of your life. This highlights the importance of creating opportunities for therapy in the other 99% of the time we are not present. By using a capacity building approach we can educate, coach and model to the people you see everyday (support workers, family, peers, teachers, etc) so they can continue the strategies learned in therapy sessions. This allows translation of new skills into your everyday routine in line with evidence based practice. Using this approach, it means that it is unlikely you will only ever have 1:1 sessions with your Occupational Therapist at the same time every week/fortnight. Instead, each session will be intentionally planned to ensure therapy occurs in the right environment, with the right people present.
We will ask a range of questions to ensure we understand what is important to you and determine whether we are the right provider to support you achieve your goals.
We come to you, preferably to your home, as this maximises our understanding of your routines. Given that we spend significant amounts of time at our own homes, home is where most therapy will occur on daily basis and allows us to work with the resources you have. The initial assessment session aims at understanding your history, preferences, values, goals, and allows us to understand how we can work together. You can invite important people along to this meeting that can assist in explaining your story. We will sign our service agreement together during this session.
More often than not, follow up sessions will be required to gather detailed subjective and objective data (using semi-structured interviews, observations, and use of standardised assessment tools). We may need to complete sessions in different environments e.g day program, work, community. During this phase we also collaborate with other members of your support team (with your consent) to ensure we gather a holistic picture of your circumstances.
Once we have a good understanding of your long-term goals, objectives and aspirations, and we have finalised the assessment process to determine your functional capacity, we can now identify the functional outcomes that can be expected to be achieved within the plan period and the funding available. We will identify the strategies and actions needed to achieve these expected outcomes in line with evidence based practice and we will share our findings with you. This session is a collaborative discussion and allows all parties to understand each persons role in achieving the desired outcomes, ensuring we work together as one.
This is where the magic happens and you will start to benefit from therapy. This is implementing the therapy plan.
We complete regular checks ins focused on evaluate the effectiveness of therapy intervention. We review the goals set, determine if changes need to be made to the therapy plan, monitor funds available within our service agreement to prevent over or under utilisation of your funding. During this time, we will ask you for feedback and encourage you to let us know what is working and not working, so we can adapt our approach to better support you.
We strive to build capacity and share our clinical recommendations to people we support via discussion, as this will allow for genuine progression towards understanding the steps needed to achieve your goals. There may be some situations where written reports are necessary, and this is usually for the benefit of satisfying NDIS funding requirements (e.g scheduled or unscheduled plan reviews, or applying for assistive technology). For scheduled plan reviews, NDIS requires evidence of what outcomes were achieved during the plan period, how the funding was spent, barriers to progress, and recommendations for the next plan period. For unscheduled reviews where it has been identified the current funding is insufficient to achieve your outcomes, we can prepare more thorough evidence.
